Privacy Policy

Collaborative Care Interactive, LLC, doing business as Collaborative Care Intelligence ("CCI," "we," or "us") respects the privacy of every visitor to this site. This policy describes what information we collect, how we use it, and the choices you have.

Scope

This site is intended for a U.S. audience. We extend core privacy rights — access, correction, and deletion — to all visitors regardless of location. If you are in the EU or UK, we process inquiries on the basis of legitimate interest or your consent, and you may contact us to exercise rights granted under the General Data Protection Regulation (GDPR).

This website does not collect, store, or transmit Protected Health Information (PHI) under HIPAA. Any future engagement that involves PHI would be governed by a separate agreement.

What we collect

The only personal information we actively collect is what you submit through the contact form on this site. That submission may include:

  • Your name and email address
  • Your organization (optional)
  • The nature of your inquiry, the domain it concerns, the project type, your stage of planning, and budget range
  • Any free-text message you choose to include

If you subscribe to the essays, we collect your email address for that purpose alone, and your IP address is recorded at the moment you sign up, together with the date and the date you confirm. Those three together are the record that you asked to be on the list; they are kept for that purpose and are not used to track you. Subscribing requires you to confirm by clicking a link we email you, and every essay carries a one-click unsubscribe link; unsubscribing removes your address from the list.

The site may also passively log standard web analytics data — IP address, browser type, pages visited, referring source — for routine site operation and improvement. We do not currently use analytics services that build cross-site advertising profiles.

How we use what we collect

Form submissions are used to evaluate fit, respond to your inquiry, and discuss potential engagements. We do not sell, rent, or share your contact information with third parties for their marketing.

How it is stored and protected

Form submissions are processed by our form-handling provider and stored in our records system. Data in transit is encrypted using TLS. Access to stored data is restricted to the principal of CCI. Third-party processors are required to maintain appropriate security measures.

We retain inquiry data for 24 months from the date of last contact unless an active engagement exists, after which we delete or anonymize it. You may request earlier deletion at any time.

If a breach affecting your information occurs, we will notify affected individuals as required by applicable law (including the New York SHIELD Act) without unreasonable delay.

Third-party processors

We use third-party services for form handling, email delivery, and hosting. If you subscribe to the essays, your email address is stored in our own database, hosted by Supabase, and messages are delivered by Resend. It is used only to send you the essays you signed up for. We do not use a third-party newsletter platform, and your address is not held by one. Each processor is bound by its own data-protection terms, and we limit the data shared to what is necessary for the service to function.

Cookies and similar technologies

This site uses minimal browser storage limited to standard session-handling and accessibility preferences. We do not currently use third-party advertising cookies or cross-site tracking. If we add analytics cookies in the future, we will request your consent before placing them.

Your choices

You may request a copy of the information we hold about you, ask us to correct it, or ask us to delete it, by contacting us through the form on this site or by writing to Collaborative Care Interactive, LLC. We will respond within a reasonable time.

We will honor deletion requests unless we need to retain the data to comply with a legal obligation, resolve disputes, or enforce agreements. In that case, we will inform you of the reason.

The methodology, applications, written content, and visual design of this site are the property of Collaborative Care Interactive, LLC unless otherwise noted. You may share short excerpts with attribution. You may not republish, repurpose, or claim authorship of substantial portions of the content without written permission.

Inquiries

By submitting an inquiry through the contact form, you give us permission to use the information you provide to evaluate fit and respond to you. You may withdraw this permission and request deletion at any time.

Site is provided "as is"

We make reasonable efforts to keep this site accurate and operational, but we provide it "as is" without warranties of any kind. We are not liable for any consequences arising from your use of the site or reliance on its content.

Governing law and disputes

These terms are governed by the laws of the State of New York, without regard to conflict-of-laws principles. Any dispute arising out of or relating to this site or these terms shall be resolved in the state or federal courts located in New York, and you consent to the personal jurisdiction of those courts.

Changes

We may update these terms from time to time. The updated version will be posted on this page with a new effective date.

Collaborative Care Intelligence is committed to making this site usable by the broadest possible range of visitors, including those who use screen readers, keyboard navigation, or other assistive technologies.

Standards we aim for

This site is designed with the goal of meeting the Web Content Accessibility Guidelines (WCAG) 2.1, Level AA. We test against this standard and address gaps as we find them.

Known limitations

This site uses canvas-based animation and progressive-reveal forms. We have made reasonable accommodations for assistive technology but acknowledge that some interactions may be more usable with a mouse than with a keyboard or screen reader. If you encounter a barrier, please let us know.

How to report a problem

If you encounter an accessibility issue on this site, please contact us through the form. We will make reasonable efforts to address the issue or to provide the relevant information by alternative means.
