Decades of pain science confirm that psychological and social factors — attitudes, beliefs, fear of movement, catastrophizing, self-efficacy — can have a greater impact on function and disability than the initial injury itself. A patient's beliefs about their pain can be as diagnostically significant as an MRI. Yet standard assessments capture none of this. They reduce the experience to a score and miss the patterns that actually matter.

PainCoach was built to close that gap. It operationalizes the biopsychosocial model by transforming a static questionnaire into an adaptive coaching conversation that reveals not just what you're experiencing, but how you think about your pain, what you believe about your future, and where you are in your readiness to change.

Fear-Avoidance

Rest feels safe but creates a cycle: avoidance increases disability, which reinforces fear. PainCoach identifies where you are in that cycle and helps you find your way out.

Self-Efficacy

Your belief in your ability to manage pain is one of the strongest predictors of recovery. Building that confidence is not a side effect of treatment — it's the goal.

Readiness to Change

You might be ready to manage medication but avoiding exercise out of fear. PainCoach assesses readiness per domain, not as a single global score.

PainCoach delivers questions one at a time through a conversational interface with an adaptive coach. After assessment, it transitions into a coaching dialogue that probes deeper into the patterns the assessment revealed — mapping your responses to stages of readiness to change, not globally, but for each specific behavior being discussed.

Each phase builds on the previous one. Assessment data informs the coaching dialogue. Coaching responses feed the provider report. The entire session is stored for longitudinal tracking — so both you and your provider can see how your attitudes, readiness, and self-efficacy evolve over time.

The assessment is the intervention

Most digital pain tools treat psychosocial assessment as an optional add-on — a depression screener appended to a pain questionnaire. PainCoach treats it as the foundation. The entire system — assessment categories, coaching dialogue, clinical alerts, readiness profiles — is architected around the premise that understanding how you think about your pain is as important as understanding how it feels.

When patients feel heard and understood, they become more engaged partners in their care. When they set and achieve small, meaningful goals, they build self-efficacy — the belief in their ability to manage their pain. That sense of control is the most powerful antidote to helplessness.

PainCoach identifies yellow flags — the psychosocial risk factors that pain science recognizes as predictors of chronic disability: catastrophizing, fear-avoidance beliefs, low self-efficacy, emotional distress, and treatment skepticism. Identifying these early enables targeted intervention rather than assumptions about motivation.

Built from clinical reality, not theory

PainCoach didn't start as a product concept. It grew out of years of applied knowledge engineering work with physical therapists, spine rehabilitation specialists, and pain clinicians — mapping how expert practitioners actually assess and communicate with their patients.

Clinical Knowledge Engineering

Extensive work with rehabilitation practitioners to capture the clinical reasoning patterns that distinguish expert pain assessment from checklist-driven evaluation. The focus wasn't on what questions to ask, but on how experienced clinicians interpret the answers.

Biopsychosocial Research

Deep integration with the pain science literature — particularly the evidence that psychosocial factors are among the strongest predictors of rehabilitation outcomes. This research became the foundation for PainCoach's three-category assessment model: provider communication, activity management, and pain acceptance.

Behavioral Coaching Design

Drawing on the science of behavior change — stages of change theory, motivational interviewing, cognitive-behavioral techniques, and related evidence-based frameworks — to create a coaching dialogue that adapts to where the patient actually is, not where the system assumes they should be.

AI-Enhanced Reimagination

Now being rebuilt with AI capabilities that enable truly adaptive coaching conversations, natural language processing for richer patient interaction, and intelligent pattern recognition across longitudinal data. The clinical knowledge stays the same. The technology finally matches the ambition.
